Services begin with a referral from a support coordinator or case manager. Once authorization is approved, we can get started.
Assessment
We complete an initial assessment to better understand the individual’s strengths interests, needs, and goals.
Personalized Support Plan
A customized, recreation therapy–based plan is developed with targeted strategies to support skill development and independence.
Training & Implementation
We provide hands-on training for caregivers, staff, and support team members, ensuring strategies can be confidently used across home, community, and program settings.
Ongoing Collaboration & Coaching
Through continued communication, observation, and coaching, we support consistency, progress, and meaningful skill carryover into everyday life.
